Job Title: Pharmaceutical Care Specialist

We are seeking a highly skilled Pharmaceutical Care Specialist to join our team at Novant Health.

About the Role:

  • Deliver exceptional patient care by providing optimal medication management in oncology.
  • Partner with healthcare providers and nursing teams to support direct patient care.
  • Participate in clinical trials and new drug services.

Responsibilities:

  1. Conduct clinical reviews, order verification, and patient counseling to ensure accurate medication administration.
  2. Facilitate pharmaceutical care services and medication utilization systems to align with patient needs.
  3. Maintain critical patient monitoring and review patient profiles/charts to identify potential drug-related issues.
  4. Provide evidence-based, patient-centered medication recommendations.

Requirements:

  • 4-year Bachelor's Degree in Pharmacy or Doctorate of Pharmacy preferred.
  • Postgraduate year 1 residency or fellowship or two years of experience as a registered pharmacist in health system pharmacy required.
  • Hematology/Oncology experience strongly preferred.
  • Current Pharmacy licensure in the state(s) where you will work or must be obtained within 30 days of start date.
  • Board of Pharmacy Specialties Certification or other approved certification must be obtained and continuously maintained within 2 years of start date.
