Hospice Nurse Practitioner PPV Specialist

At Compassus, we are seeking a highly skilled and compassionate Hospice Nurse Practitioner PPV to join our team. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to individuals and their families in need of palliative and hospice services.
Responsibilities:- Provide routine and emergency assessments, recertification visits, and educational services to patients and their families.
- Collaborate with the interdisciplinary team, including physicians, nurses, social workers, and chaplains, to develop and implement individualized care plans.
- Assess and manage symptoms, provide disease and treatment education, and promote patient and family coping.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Document patient information, including assessments, treatments, and outcomes, in a timely and accurate manner.
- Participate in ongoing education and professional development to stay current with best practices in hospice and palliative care.
- Master's degree in Nursing required.
- Active and unencumbered CNS or Nurse Practitioner license in state of employment required.
- Certified as a Nurse Practitioner in Adult, Family, Acute, or Geriatric required.
- Active NPI number.
- Maintain active DEA certification if state allows prescription of controlled substances by APPs.
- Board Certification in Hospice and Palliative Care is preferred, but not required.
