Hospice Nurse

Job Summary
Enhabit Home Health & Hospice is seeking a part-time Hospice RN to join our team. As a Hospice RN, you will provide skilled nursing visits and coordinate client care to patients in their homes.
Responsibilities
- Perform skilled nursing visits and complete coordination of client care
- Point of contact for all disciplines involved with providing care to patients
- Oversee the frequency of visits for the episode
- Consult with the physician and office staff as needed
- Attend weekly case conference and monthly case manager meetings
Requirements
- Must be a graduate of an approved school of clinical education
- Must be licensed in the state where they currently practice
- Must have at least one year experience as a licensed professional
- Must have basic demonstrated technology skills, including operation of a mobile device
Preferred Qualifications
- Experience as a licensed professional may be deferred with a review and approval from the regional and executive service line clinical leadership
- Medicare home health or hospice experience is preferred
