Hospice Care Specialist

We are seeking a compassionate and skilled Hospice Nursing Assistant/CNA to join our team at Enhabit Home Health & Hospice. As a Hospice Nursing Assistant/CNA, you will play a vital role in providing high-quality patient care and support to individuals and their families in their homes.
Responsibilities- Assist patients with various personal care activities, including bathing, dressing, and grooming.
- Provide emotional support and companionship to patients and their families.
- Collaborate with the hospice team to develop and implement individualized care plans.
- Consult with supervising registered nurses to ensure seamless care coordination.
- Minimum of 1 year of experience in direct client care in an institutional setting or home health/hospice setting.
- Valid state driver's license and reliable transportation.
- Ability to recognize patient needs, maintain good relationships, and communicate effectively.
