Opportunity for a Case Management Specialist at Reach for Resources

Position Overview:

  • Deliver case management services for individuals eligible for CADI or BI Waiver programs.
  • Support clients in accessing essential services that cater to their social, recreational, health, educational, and vocational needs.
  • Oversee a caseload of up to 45 clients, ensuring personalized attention and care.

Primary Duties:

  • Provide case management in accordance with DHS and county regulations.
  • Conduct regular meetings with clients and facilitate necessary referrals.
  • Ensure confidentiality and maintain precise documentation of client interactions.
  • Implement community-based services centered around the individual’s needs.
  • Engage in networking and public relations activities to enhance service delivery.
  • Effectively utilize resources to connect clients with appropriate services.
  • Collaborate with colleagues and participate in agency initiatives as needed.

Role Scope:

The case management specialist will work closely with clients, families, and support networks to deliver vital mental health, social, educational, and vocational services. Responsibilities include assessing client needs, crafting individualized plans, linking to community resources, and monitoring service effectiveness.

Qualifications:

  • Bachelor's degree in social work, psychology, sociology, or a related field.
  • Experience in supporting individuals with disabilities or mental health challenges.
  • Ability to work autonomously, manage stress effectively, and pass a background screening.
  • Possession of a valid driver's license, reliable transportation, and adequate auto insurance.

  • Proficient in communication, report writing, and crisis intervention.
  • Experienced in developing treatment plans and working with diverse populations.
  • Proficient in computer applications, particularly Microsoft Office.
