Certified Peer Specialist/Case Manager

River Edge Behavioral Health is seeking a Certified Peer Specialist/Case Manager to join our team. As a Certified Peer Specialist/Case Manager, you will play a crucial role in providing support to individuals on their recovery journey.
Job Summary:The Certified Peer Specialist/Case Manager will work with a multidisciplinary team to provide support to individuals with mental health and substance use disorders. The ideal candidate will have a strong understanding of the Housing First and Harm Reduction models and be able to provide peer support and case management services to individuals in need.
Responsibilities:- Provide peer support and case management services to individuals with mental health and substance use disorders.
- Work with a multidisciplinary team to provide comprehensive services to individuals in need.
- Develop and implement individualized service plans to meet the unique needs of each client.
- Provide referrals for comprehensive services to ensure housing sustainability.
- Maintain accurate and up-to-date records of client interactions and progress.
- Collaborate with outside agencies and referral resources to ensure seamless service delivery.
- Bachelor's degree in social service field (Social Work, Sociology, Psychology, Human Services) or related field.
- Certified Peer Specialist certification or equivalent experience.
- 1 year of experience in case management or peer support services.
- Knowledge of the Housing First and Harm Reduction models.
- Proficiency with Homeless Information Management System, Client Track, or similar software.
