Job Summary

We are seeking a skilled Registered Nurse (RN) to join our team at IntelyCare. As a Nurse Care Specialist, you will provide high-quality care to patients in post-acute facilities, working closely with physicians and other medical staff to develop and implement effective care plans.

Key Responsibilities
  • Provide quality care to patients in post-acute facilities, including administering treatments and medications, and monitoring medical equipment.
  • Develop and implement care plans in collaboration with medical staff and residents, ensuring the best possible outcomes.
  • Supervise and train nursing and other staff, as needed, to ensure compliance with nursing standards and facility policies.
  • Document patient histories and interventions accurately and thoroughly.
  • Participate in incident investigations and contribute to a safe and healthy work environment.
Requirements
  • Current Connecticut state license to practice as a Registered Nurse (RN).
  • Diploma from an approved nursing program.
  • At least 4 months of post-acute or in-patient hospital care experience.
  • Excellent organizational and supervisory skills.
