Job Summary

Key Responsibilities
  • Provide quality care to patients in post-acute facilities, including administering treatments and medications, and monitoring patient progress.
  • Develop and implement care plans in collaboration with physicians and other medical staff, ensuring that patients receive the best possible care.
  • Supervise and train nursing and other staff members, ensuring that they are equipped to provide high-quality care.
  • Document patient histories and interventions, and participate in incident investigations as needed.
  • Work with residents and family members to ensure that patients receive the best possible care, and provide support and guidance as needed.
Requirements
  • Current Connecticut state license to practice as a Registered Nurse (RN).
  • Diploma from an approved nursing program.
  • At least 4 months of post-acute or in-patient hospital care experience.
  • Excellent organizational and supervisory skills.
