About the FPATH ProgramAt Community House Mental Health Agency, we offer a comprehensive community-based supported housing program for adults with severe and persistent behavioral health issues. Our Forensic Projects for Assistance in Transition from Homelessness (FPATH) team provides free companion-based case management to help individuals connect to resources, ultimately breaking their cycle of incarceration.

Job Responsibilities
  • Develop meaningful relationships with individuals who face extreme barriers to engaging in services.
  • Assist clients in developing essential life skills, such as meal preparation, laundry, and hygiene.
  • Transport clients to appointments and provide immediate crisis intervention and resolution when needed.
  • Complete risk assessments and help clients build de-escalation skills and strategies.
  • Coordinate with other relevant professionals, including behavioral health, legal, medical, and more.
  • Support the F-PATH Emergency Housing intake and interview process.
  • Help clients follow up on legal expectations to reduce recidivism.
  • Engage with clients under the influence of substances and those experiencing poverty and poor hygiene.
  • Implement a harm reduction approach, demonstrating passion for serving individuals with co-occurring mental health and substance use disorders at various stages of change.
  • Advocate for clients and assist them in learning self-advocacy skills.
  • Meet timely documentation requirements to program standards.
  • Maintain confidentiality and adhere to HIPAA standards.
Requirements
  • Bachelor's degree in a social services-related field.
  • Legible handwriting.
  • A valid driver's license and clean driving record for agency vehicle use.
  • Experience working with individuals with severe and persistent mental illness, chronic substance use issues, and involvement in the criminal justice system is preferred.
  • Strong diagnostic and assessment skills, along with expertise in Co-Occurring disorders.
  • Familiarity with the forensic/competency system or background.
  • Ability to work non-traditional hours, including holidays.
  • Basic understanding of the criminal justice system.
  • Sensitivity to diverse populations and cultural awareness.
  • Eligibility for and ability to obtain Washington State Agency Affiliated Counselor License.
  • Capacity to perform job duties within reasonable accommodation.
