POSITION SUMMARY:
The Care Services Coordinator plays a crucial role in supporting families affected by ALS.
This position is designed to enhance the functional capabilities of both patients and caregivers, serving as a vital link between healthcare professionals and community resources to guarantee timely and effective care.
KEY RESPONSIBILITIES:
Program Implementation and Evaluation:
- Execute care management strategies aimed at improving the quality of life for patients.
- Oversee client caseloads, formulate care plans, and coordinate clinic assistance.
- Assess client requirements, create care plans, and advocate for essential services.
- Collaborate with ALS Treatment Centers to ensure seamless care continuity.
Support Group Facilitation:
- Organize and lead support group sessions for clients and their families.
- Arrange educational workshops and invite knowledgeable guest speakers.
Administrative Responsibilities:
- Ensure adherence to organizational policies and maintain accurate client records.
- Generate reports, correspondence, and memos utilizing Microsoft Office applications.
Community Education and Outreach:
- Implement educational initiatives for the community.
- Advocate for ALS Association events and programs.
- Maintain professionalism and engage effectively with diverse stakeholders.
QUALIFICATIONS:
- Bachelor’s degree in public health, social work, nursing, or a related discipline.
- At least 3 years of experience in healthcare or community service settings.
- Familiarity with Medicare, Medicaid, and case management is preferred.
- Proficient in Microsoft Office and database management systems.
- Excellent communication and interpersonal abilities.
COMPENSATION:
The ALS Association offers a competitive salary for this role, with an hourly pay rate of $27.99. The organization provides a comprehensive benefits package, including healthcare coverage, a 401(k) retirement plan, disability insurance, and paid time off.
