We are seeking a highly skilled Physician to join our team at CenterWell Senior Primary Care. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and contributing to the development of our care model.
Key Responsibilities- Provide proactive, preventive care to seniors, including wellness visits, physical exams, and chronic condition management.
- Work collaboratively with our integrated care teams to address the physical, emotional, and social wellness of our patients.
- Develop and implement personalized care plans that meet the unique needs of each patient.
- Stay up-to-date with the latest medical research and best practices to ensure the highest quality of care.
- Contribute to the development of our care model and provide feedback to improve patient outcomes.
- Doctorate degree in medicine or osteopathic medicine.
- 5 or more years of experience in value-based care.
- Medical licensure requirements for the state of Florida.
- Graduate of an accredited MD or DO program.
- Board certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Bilingual or multilingual skills.
- Experience working in a senior-focused primary care setting.
