Position Summary:
We are looking for a compassionate and skilled On-Call Overnight Hospice Nurse to provide exceptional nursing care and emotional support to patients and their families during the night shift, weekends, and occasional holidays. This role demands flexibility, empathy, and the ability to offer comfort and reassurance during difficult times.
Key Responsibilities:
- Deliver skilled nursing care to hospice patients in their residences or inpatient settings during overnight hours.
- Evaluate patients' physical and emotional requirements and create personalized care plans in collaboration with the interdisciplinary team.
- Administer prescribed medications, treatments, and procedures as directed by the attending physician.
- Monitor patients' conditions and vital signs, reporting any changes or concerns to the appropriate healthcare professionals.
- Provide emotional support and guidance to patients and families, addressing their concerns and offering comfort during challenging times.
- Educate patients and families on hospice philosophy, pain management, symptom control, and end-of-life care options.
- Maintain accurate and comprehensive documentation of patient care activities and communication with the interdisciplinary team.
Qualifications:
- Current RN or LPN license in Georgia.
- A minimum of 2 years of nursing experience, preferably in hospice, palliative care, or a related field.
- Certification in Hospice and Palliative Care Nursing (CHPN) is preferred but not mandatory.
- Excellent communication and interpersonal skills, with the ability to effectively interact with patients, families, and interdisciplinary team members.
- Strong critical thinking and problem-solving skills.
- Ability to work independently and make sound judgments in emergency situations.
- Willingness to work overnight shifts, weekends, and occasional holidays as required.
