Position Overview:
We are in search of a compassionate and dedicated On-Call Overnight Hospice Nurse to provide exceptional nursing care and emotional support to terminally ill patients and their families during night shifts, weekends, and occasional holidays. This role requires flexibility, empathy, and the ability to provide comfort and reassurance during challenging times.
Key Responsibilities:
- Deliver skilled nursing care to hospice patients in their homes or inpatient facilities during overnight hours
- Assess patients' physical and emotional needs and develop individualized care plans in collaboration with the interdisciplinary team
- Administer medications, treatments, and procedures as prescribed by the attending physician
- Monitor patients' conditions and vital signs, promptly reporting any changes or concerns to the appropriate healthcare professionals
- Provide emotional support and guidance to patients and families, addressing their concerns and offering comfort during difficult times
- Educate patients and families on hospice care principles, pain management, symptom control, and end-of-life care options
- Maintain accurate and detailed documentation of patient care activities and communications with the interdisciplinary team
- Valid RN or LPN license in Georgia
- Minimum of 2 years of nursing experience, preferably in hospice, palliative care, or related fields
- Hospice and Palliative Care Nursing Certification (CHPN) is preferred but not required
- Excellent communication and interpersonal skills for effective interaction with patients, families, and team members
- Strong critical thinking and problem-solving abilities
- Ability to work independently and make informed decisions in emergency situations
- Willingness to work overnight shifts, weekends, and occasional holidays as needed
