**Key Responsibilities:**

* Lead a team of skilled nurses in delivering high-quality patient care
* Collaborate with healthcare professionals to develop and implement effective care plans
* Foster a culture of empathy, understanding, and open communication
* Ensure seamless transitions and continuity of care
* Participate in ongoing education and training to stay up-to-date on best practices

**Requirements:**

* 5 years of professional nursing experience, with a focus on medical, surgical, ICU, palliative care, or oncology
* 2 years of community health, home health, or hospice and palliative care nursing experience preferred
* 1 year of clinical supervisory experience preferred
* Active, unencumbered state RN license
* Current driver's license and auto liability coverage
* CPR certification, TB Results, and updated physical
