Position Overview:
  • The Treatment in Motion (TIM) Supportive Services Coordinator is an integral part of the Treatment in Motion program team, contributing to ETS' mission by building trusting relationships with program participants as they pursue their goals and receive ongoing support to enhance their stability and quality of life.
  • This role significantly impacts ETS by providing outreach, engagement, case management, and care coordination services to adults facing homelessness and behavioral health challenges, particularly those diagnosed with opioid use disorder. The focus is on assisting individuals whose complex needs hinder their engagement in Medication for Opioid Use Disorder (MOUD) treatment.
  • This is a full-time onsite position, with responsibilities spanning various outreach locations.
  • The compensation for this role ranges from $28.00 to $30.00 per hour. (The annual equivalent is $58,240 - $62,400.) ETS is committed to achieving pay equity and transparency as part of our equitable workplace initiative.
  • Work hours are from 5:00 AM to 1:30 PM, Monday through Friday, with rotating Saturday shifts.
Key Responsibilities:
  • Collaborate with fellow staff to help ETS achieve its mission by transforming systems of harm and inequity, fostering innovative approaches to enhance community health and safety through addressing substance use and homelessness.
  • Support the organization-wide initiative to reimagine our interconnectedness within the community, working to dismantle societal and organizational structures influenced by white supremacy and settler colonialism.
  • Provide outreach, engagement, intensive case management, and care coordination services for assigned participants:
  • Meet participants in various settings to explain available services and establish a working relationship aimed at facilitating their entry into MOUD treatment through the TIM program.
  • Build rapport and trust with participants through respectful outreach and engagement practices centered on their needs.
  • Collaborate with participants to develop and implement an individualized service plan (ISP) that addresses their specific needs and priorities, regularly reviewing and updating the plan to reflect progress and emerging needs.
  • Assist participants in planning for consistent attendance in treatment activities as outlined in the ISP.
  • Support participants in securing and maintaining stable housing whenever possible.
  • Help participants establish and maintain primary medical care while addressing chronic health conditions.
  • Assess the nature and severity of participants' substance use issues, aligning clinical interventions with their stage of change to mitigate the harmful consequences of substance use.
  • Facilitate access to mental health and substance use disorder treatment services, ensuring participants engage with appropriate supportive services.
  • Assist participants in achieving financial stability through access to entitlement programs, protective payee-ships, educational and vocational programs, and employment opportunities.
  • Develop and maintain productive relationships with community programs that can support participants, ensuring effective care coordination.
  • Deliver services in accordance with ETS/REACH program policies and procedures.
  • Support participants in keeping appointments and developing spending plans.
  • Advocate for participants with various service providers, assisting them in accessing necessary services by attending appointments together to promote continuity of care.
  • Identify gaps and barriers in community resources and advocate for systemic changes.
  • Participate in TIM team meetings and other required gatherings.
  • Utilize clinical supervision and peer support to explore intervention strategies, resource development, and self-care.
  • Maintain participant files for assigned caseload in compliance with agency, contract, and state requirements.
  • Local travel is a requirement of this position.
  • Assist with driving program vehicles and ensuring site-based safety as needed.
  • Contribute insights for ongoing quality improvement to enhance the participant experience.
Qualifications:
  • High School Diploma required; undergraduate degree preferred.
  • Academic training in social services and substance use disorder/mental health treatment is desirable.
  • SUDP/SUDPT and/or MSW is highly preferred.
  • A minimum of two years of experience in substance use disorders, social work, or outreach programs is required.
  • Individuals with lived experience relevant to our patient population are highly valued.
  • Experience providing harm reduction services to individuals facing behavioral health challenges is highly desirable.
Knowledge and Skills:
  • Understanding of racial justice and social equity, with a commitment to fostering an equitable environment for all clients and staff.
  • Ability to establish effective working relationships with clients, patients, and staff from diverse backgrounds.
  • Possession of a valid driver's license.
  • Strong interpersonal and communication skills, both verbal and written.
  • Excellent organizational skills with the ability to prioritize tasks and work independently.
  • Dependable and adaptable, with a cooperative approach to problem-solving.
  • Team player with attention to detail.
  • Ability to maintain confidentiality and handle sensitive information discreetly.
  • Skills in setting boundaries, resolving conflicts, and de-escalating situations.
  • Proficient in Microsoft Office Suite and willing to keep up with technological advancements.
Physical Requirements:
  • Exposure to illicit drug residues and potential biohazardous materials is possible in this role. Training will be provided to mitigate risks.
  • Regularly required to be on feet, with tasks including standing, walking, and kneeling, and may involve working outside in various weather conditions.
  • Occasional lifting of up to 30 pounds may be required.
