Job Summary

The Episcopal Community Services Of San Francisco is seeking a highly skilled and compassionate Interim Housing Case Manager to join our team. As a key member of our organization, you will be responsible for managing a caseload of up to 25 participants at our Sanctuary Shelter in San Francisco, CA.

Key Responsibilities
  • Manage a caseload of up to 25 participants, providing individualized support and services to help them achieve their goals.
  • Perform comprehensive assessments and develop individualized service plans to address the unique needs of each participant.
  • Collaborate with program leadership to ensure compliance with county, state, and federal regulations.
  • Provide back-up coverage for other case managers as needed.
  • Develop and implement action plans to assist participants in achieving their goals.
  • Document participant progress and maintain accurate records in compliance with ECS practices.
  • Provide linkage services and support for housing, income, primary care, substance use treatment, behavioral health, legal, employment, clothing, and other resources.
  • Meet with participants at minimum once a week and document sessions.
  • Track and encourage participants to attend scheduled appointments.
Requirements
  • Associate degree in psychology, counseling, social work, or a related field and 3 years of relevant work experience.
  • CCSF Community Health Worker Certificate and 2-4 years of relevant work experience.
  • High school diploma/GED and 4 years of relevant work experience.
  • Experience working in integrated service teams using person-centered harm-reduction and trauma-informed approaches.
  • Strong background working with individuals experiencing homelessness.
  • Effective communication skills (written and oral).
  • Excellent organizational and time management skills.
  • Strong administrative and organizational skills, including charting, recordkeeping, and professional writing.
  • Knowledge of Microsoft Office programs and experience documenting services into electronic record systems.
  • Ability to problem solve and quickly assess/respond to a variety of situations, including medical emergencies, psychiatric emergencies, conflict resolution, and de-escalation interventions.
  • Cultural humility and ability to work in a diverse program and staff population.
  • Excellent working knowledge of homelessness resources in San Francisco.
  • Strong knowledge of and sensitivity to adults with substance abuse, domestic violence, incarceration, HIV/AIDS, mental health, including co-occurring disorders, and history of trauma.
  • Ability to welcome and respect co-workers and participant populations as unique individuals with different backgrounds, perspectives, and personalities.
