Housing Case Manager

The Unity Care Group is seeking a skilled Housing Case Manager to join our team. As a key member of our Navigation Program, you will provide direct support and case management services to clients, ensuring they achieve their personal and professional development goals.
Key Responsibilities- Build relationships with clients, ensuring they complete and sign required documentation, and work with the San Francisco Housing Authority to acquire housing vouchers.
- Manage a caseload of up to 50 scattered-site young adult clients, developing a relationship and maintaining individual contact with each participant at least monthly to discuss goals and track progress.
- Maintain ongoing contact with clients' support systems, including natural supports, family, referring agencies, and community resources.
- Participate in relevant program and City team meetings, serving as a representative of UCG and liaison with these parties.
- Support clients to develop case plans, including individualized goals, and maintain current documentation in Awards and other program data entry systems.
- Bachelor's Degree in social work or another human services field, or three years' experience working with system-involved youth from foster care, juvenile probation, or similar settings.
- Knowledge of Microsoft Office applications and database systems.
- Demonstrated skills in case work, group work, and case coordination.
- Excellent interpersonal skills (verbal and written).
- Demonstrated ability to facilitate and link relevant services to address resident/client needs.
- Ability to maintain a positive perspective in the treatment and care of young adults with mental, emotional, and social issues.
- Master's degree in a social work or related human service field.
- A minimum of two years of experience working with system-involved youth from foster care, juvenile probation, or similar settings.
- Experience working in a scattered-site housing program with case management services.
