Job Title: Quality Improvement Facilitator

The Quality Improvement Facilitator plays a pivotal role in guiding primary care and specialty practices towards achieving certified NCQA Patient Centered Medical Homes. This grant-funded position works closely with the Rutland Health Service area Primary Care Practices engaged in Vermont's Blueprint for Health. The facilitator assists healthcare teams in assessing, prioritizing, and implementing process improvements, as well as evaluating their effectiveness. Collaboration with local Blueprint Teams and Accountable Care Organizations (ACOs) is essential in supporting community-based Quality Improvement efforts.

Key Responsibilities:
  • Assess and prioritize process improvements in primary care and specialty practices.
  • Develop and implement action plans to enhance quality and patient outcomes.
  • Evaluate the effectiveness of implemented improvements.
  • Collaborate with local Blueprint Teams and ACOs to support community-based Quality Improvement initiatives.
Requirements:
  • Bachelor's degree in a healthcare-related field.
  • Master's degree preferred.
  • 5 years of experience in a facilitator or related healthcare role, with 3 years leading clinical quality improvement initiatives.
  • Prior experience working within Clinical Microsystems framework desirable.
Preferred Qualifications:
  • Quality Certification (CPHQ, CHQ, or HCQM) highly desirable.
  • Lean or Six Sigma Certification preferred.
  • Maintains a valid Vermont state driver's license.
  • Current Vermont professional licensure (as applicable).
Salary Range:
