UnitedHealth Group Inc is seeking a Senior Clinical Program Manager for Chronic Care Management to join our team in Houston, TX.

About the Role

This is an exciting opportunity to work with our talented team of clinicians and professionals who are dedicated to improving the health and well-being of seniors. As a Senior Clinical Program Manager, you will be responsible for leading the development and implementation of clinical programs that aim to improve patient outcomes and reduce healthcare costs.

Responsibilities
  • Develop and implement clinical programs for chronic care management
  • Collaborate with multidisciplinary teams to identify patients at high risk and develop individualized care plans
  • Provide education and support to patients and their families on self-management techniques and disease prevention
  • Coordinate with providers to establish or update plan of care
  • Perform accurate and timely documentation in the electronic medical record
Requirements
  • Bachelor's degree in Nursing or Associate's degree in Nursing with 4+ years of experience as an RN
  • Registered Nurse with an active and unrestricted license to practice in the state of employment
  • Current BLS certification
  • 2+ years of experience in a physician's office, clinical or hospital setting
  • Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF and IHD
Preferred Qualifications
  • Experience related to patient education and/or motivational interviewing skills and self-management goal setting
  • Fluent written and verbal skills in English and Spanish
