Advanced Care Coordinator

We are seeking a highly skilled Advanced Care Coordinator to join our team at Monogram Health, where you will play a vital role in delivering personalized medical care to individuals with chronic kidney disease.
About the RoleAs an Advanced Care Coordinator, you will be responsible for conducting comprehensive assessments of patients, providing education and counseling on benefits and programs available to improve their quality of life, and documenting patient information accurately. You will work collaboratively with primary care providers, specialists, and other healthcare professionals to coordinate care and ensure seamless transitions between services.

RequirementsTo succeed in this role, you must have:
- An active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
- Board certification for appropriate licensure (NP: ANCC/AANP; PA: NCCPA)
- A current and unrestricted DEA certificate
- The ability to work without direct supervision and practice autonomously
- Access to transportation, a valid driver's license, and car insurance
- Proficiency with medical instruments and equipment required by the work
- Knowledge of computer-based data management programs and information systems, as well as medical records and point-of-interview technology
- The ability to communicate effectively in verbal and written form with patients, family members, physicians, and representatives of the community
- Sound understanding of federal and state regulations, including HIPAA and OSHA
- At least 2 years of direct patient care experience, preferably in managed care/IPA/health plan settings
- Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home
