Position Overview

Sign-On Bonus: $5,000 for Full-time externals only, pro-rated for Part-time

Role: This is a Cardiology Medical Office and Heart Failure RN position located in a dynamic healthcare environment.

Schedule: Full-time, 40 hours per week, Monday to Friday, between 8:30 AM and 5:00 PM.

Job Summary:

The Heart Failure Nurse acts as a vital resource for patients and healthcare providers, focusing on education, direct patient care, and quality enhancement for both outpatient and inpatient services within the Congestive Heart Failure (CHF) program. Key responsibilities encompass assessment, implementation, and evaluation related to discharge education, data collection, and treatment planning aimed at promoting self-management and follow-up care to mitigate readmission risks.

Key Responsibilities:

  • Collaborate with clinical teams to optimize care management for all patients, ensuring pre-visit planning and post-visit follow-up.
  • Work alongside physician leadership to establish quality measures and develop protocols based on nationally recognized standards.
  • Manage a panel of high-acuity chronic care patients, primarily focusing on Heart Failure education and support.
  • Maintain regular follow-up with patients to ensure adherence to self-care recommendations and track compliance with appointments.
  • Provide telephone support according to established protocols and manage urgent care needs for patients.
  • Coordinate with the patient's care team to assess readiness for change and develop personalized treatment plans.
  • Assist patients in setting self-management goals and report any abnormal findings to the healthcare team.
  • Administer medications as necessary and collaborate with case managers for additional services.
  • Document patient interactions and maintain records for CHF patients effectively.
  • Exhibit Nuvance Health values and fulfill compliance responsibilities related to the role.
  • Demonstrate consistent and reliable attendance.
  • Perform additional duties as required.

Education and Experience Requirements:

  • Minimum of three (3) years of experience in Acute Care nursing or equivalent.
  • Associate's Degree in Nursing required; Bachelor's Degree in Nursing preferred.

Knowledge, Skills, and Abilities:

  • Proficient in computer skills, including data entry and Microsoft Office applications.
  • IV insertion skills are essential.

Licensure and Certification:

  • Current NYS RN License, ACLS, and BLS certifications are required.
