We are seeking a highly skilled and experienced RN Cardiology Specialist to join our team at Nuvance Health. As a key member of our cardiovascular team, you will play a critical role in providing exceptional patient care and support to patients with heart failure.
Key Responsibilities- Assess and implement individualized treatment plans for patients with heart failure, including education and support for self-management and follow-up care.
- Collaborate with physicians and other healthcare professionals to ensure seamless care coordination and communication.
- Develop and maintain relationships with patients, families, and caregivers to provide emotional support and guidance throughout the care process.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Stay up-to-date with the latest clinical guidelines and research to provide evidence-based care.
- Minimum two (3) years' experience in acute care nursing or equivalent experience.
- Associates Degree in Nursing required - Bachelor's Degree in Nursing preferred.
- Current NYS RN License and ACLS certification required.
This role requires the ability to work in a fast-paced environment, with frequent interruptions and deadlines. The successful candidate will be able to multitask, prioritize tasks, and maintain a high level of productivity while providing exceptional patient care.
