Position Overview

CommuniCare Health Services is seeking a dedicated Registered Nurse to assume the vital role of Mobile MDS Coordinator across our skilled nursing facilities in the Midwest Division. The ideal candidate will possess a valid RN license in Ohio and have a solid background in Long Term Care and MDS processes. Experience in managing multiple facilities or senior MDS roles is highly desirable.

Key Responsibilities

  • Conduct evaluations of facility compliance with federal regulations and utilization targets.
  • Perform weekly visits to various facilities for audits, training, and intervention.
  • Review clinical records of residents to ensure proper coverage, utilization, and error-free assessments.
  • Provide expert consultation on federal and state reimbursement systems.

Qualifications

  • Current RN license in Ohio.
  • A minimum of 2 years of experience with Medicare/Medicaid and Managed Care reimbursement systems.
  • Preferred experience in Senior MDS roles or multi-facility MDS management.
  • AANAC certification is highly preferred.
  • Exceptional written and verbal communication skills.
  • Ability to manage multiple tasks and oversee operations across various facilities.
  • Current CPR certification is required.
