We have a unique opportunity for a Community Life Service Coordinator to join our team and contribute to the success of our residents and communities.

The Community Life Service Coordinator will work closely with our Community Life team to develop, coordinate, and implement support services for residents of Woodhill Homes. This includes providing mobile case management, conducting assessments of resident households, and developing and implementing Family Success Plans to assess and develop S.M.A.R.T goals and outcomes aligned with Choice metrics.

The successful candidate will have a strong understanding of community development principles and practices, as well as excellent communication, organization, and writing skills. They will be able to build and maintain positive relationships with a wide variety of stakeholders, including residents, senior staff, volunteers, interns, and community partners.
