Job Summary

We are seeking a skilled LVN Case Manager to join our team at Heritage Provider Network. As a key member of our clinical staff, you will play a vital role in coordinating patient care and ensuring seamless transitions between healthcare services.

Key Responsibilities
  • Assess patient needs and develop comprehensive care plans in collaboration with healthcare providers and interdisciplinary teams.
  • Coordinate patient care services, including referrals to specialists, hospitalizations, and post-acute care.
  • Communicate effectively with patients, families, and healthcare providers to ensure timely and accurate information exchange.
  • Monitor patient progress, identify potential issues, and implement interventions to achieve optimal outcomes.
  • Collaborate with social services and patient services coordinators to address patient needs and facilitate resource utilization.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
  • Stay up-to-date with industry developments, best practices, and regulatory requirements to ensure compliance and excellence in care delivery.
Requirements
  • Graduation from an accredited Licensed Vocational Nursing program.
  • Current California LVN license.
  • Current BLS certification.
  • Minimum of two years clinical nursing experience.
  • Knowledge of Microsoft Office programs.
