Registered Nurse Case Manager

The Enhanced Care Management (ECM) Case Manager RN plays a pivotal role in addressing the intricate clinical and non-clinical requirements of members facing the most challenging medical and social circumstances. This position operates under the guidance of the Case Management Manager and emphasizes systematic service coordination and comprehensive care management.
ECM is designed to be interdisciplinary, emphasizing a high-touch, person-centered approach, primarily delivered through direct interactions with members in their preferred environments. The case manager engages with individuals who may have chronic health issues, are experiencing homelessness or are at risk, have frequent hospital admissions, struggle with substance use, or are transitioning from incarceration.
Utilizing exceptional communication skills, the case manager ensures seamless service delivery and coordination with members, fostering continuity of care across various health and social service programs, including community-based and long-term support services (LTSS).
Key Responsibilities:
- Collaborate with the member's interdisciplinary team (ICT) to support their health and well-being.
- Engage members and their support systems to identify and prioritize their needs and goals.
- Maintain strong organizational and interpersonal skills to build effective relationships with members and healthcare providers.
- Utilize evidence-based communication techniques to enhance member engagement and support.
- Conduct comprehensive assessments and develop tailored care plans to address individual member needs.
Qualifications:
- Current California RN license is mandatory.
- A minimum of one (1) year of experience as a Registered Nurse in an acute care hospital, mental health setting, outpatient clinic, or healthcare agency.
- Proficient knowledge of nursing principles, medical terminology, and community resources.
- Ability to perform a full range of nursing duties and maintain accurate records.
- Experience in Medicare and Medi-Cal (CMS) environments is preferred.
