Position Overview

We are seeking a dedicated Youth Peer Advocate to be part of our Youth Assertive Community Treatment (ACT) Team at Hope For Youth.

As a Youth Peer Advocate, your role will encompass the following responsibilities:

  1. Operate in accordance with the established policies and procedures outlined in the program practice guide and employee manual.
  2. Comply with state regulations and relevant codes of conduct.
  3. Collaborate within a multidisciplinary team to support youth in their overall development and help them achieve their goals.
  4. Deliver support services through a variety of formal and informal methods to assist the youth.
  5. Facilitate engagement and active participation of youth in the treatment planning process, ensuring the ongoing application and reinforcement of skills.
  6. Encourage youth to access services that foster the development of skills and strategies essential for achieving their personal life objectives, enhancing self-advocacy skills, and supporting their transition into adulthood.
  7. Participate in training and professional development sessions as assigned.
  8. Attend daily team and family meetings as required.
  9. Implement agency-identified Evidence-Based Interventions tailored to meet the needs of clients.
  10. Provide transportation for clients to their designated service locations when necessary.
  11. Carry out additional tasks as needed to fulfill the requirements of clients, the program, and the organization.

Time Allocation:

60% Direct client interaction, including transportation services.

20% Documentation, report writing, and case-related meetings.

20% Participation in staff/team meetings, supervision, and training.

Qualifications:

  • Possession of a Youth Peer Advocate Credential with at least one year of experience, or the ability and willingness to apply for the credential.
  • Proficiency in computer applications, including Microsoft Office Suite, and experience with Electronic Health Records (EHRs).
  • A high school diploma or GED is required.
  • A valid NYS driver's license in good standing is essential.
  • Physical capabilities: ability to read/write reports, walk/stand/sit for extended periods, bend/stoop, kneel/squat, climb stairs, reach overhead, and perform repetitive wrist/finger movements while driving.
