Job Title:

Case Manager/Family Development Specialist

Job Summary:

Family Service League is seeking a highly motivated and experienced Case Manager/Family Development Specialist to join our team. The successful candidate will be responsible for providing case management services to youth and families, including linkage, coordination, advocacy, and mentoring. The ideal candidate will possess excellent interpersonal and verbal and written communication skills, as well as knowledge of community resources and sound judgment.

Responsibilities:
  • Conduct home visits throughout Suffolk County to provide case management services to youth and families.
  • Utilize Motivational Interviewing to facilitate change and support goal achievement.
  • Refer clients as needed for skill building services and work collaboratively with the skill builder to achieve client goals.
  • Document all client interactions and maintain accurate records.
  • Participate in an agency-wide System of Care and other collaborative projects.
Requirements:
  • High school diploma or equivalent required; Bachelor's degree in social work or related field preferred.
  • At least 2 years of experience working in human services required.
  • Valid and clean NYS Drivers License required.
