Youth Peer Advocate

Hope for Youth is seeking a dedicated and compassionate Youth Peer Advocate to join our Youth Assertive Community Treatment (ACT) Team. As a key member of our team, you will work closely with youth and their families to provide support and guidance in achieving their personal and individualized life goals.
Key Responsibilities:
- Provide support services to youth through formal and non-formal services
- Work within an interdisciplinary team to assist youth in their overall functioning and achieve desired outcomes
- Offer support necessary to ensure engagement and active participation of the youth in the treatment planning process and the ongoing implementation and reinforcement of skills
- Engage youth in services that provide opportunities for skill development and transition into adulthood
- Attend trainings and professional development as assigned
- Attend daily team meetings and family meetings as assigned
Requirements:
- Youth Peer Advocate Credential with one year experience or ability, willingness, and eligibility to apply for credential
- Computer proficiency, including Microsoft Office Suite, and experience working with EHRs
- High school diploma or GED
- Valid NYS driver's license in good standing
