Job Details:

The Visiting Nurse Association (VNA) of Manchester and Southern New Hampshire is seeking a skilled Patient Support Professional to join our team in Manchester and surrounding areas.

This comprehensive position involves delivering delegated nursing and homemaking duties, performing PT/INR checks, and reporting results according to policy. As a valued member of our team, you will have the opportunity to make a meaningful difference in the lives of patients and their families.

Key Responsibilities:
  • Deliver personalized home care services to patients/clients of all ages and families in their home or designated facility, ensuring a safe and supportive environment.
  • Collaborate with the primary nurse/therapist to reinforce teaching by reviewing personal care techniques with caregivers and patients.
Duties and Responsibilities:
  1. Document patient/client needs and responses to Case Manager and/or Clinical Manager, facilitating timely communication and follow-up.
  2. Report patient/client changes in condition or situations immediately, prioritizing prompt attention and intervention.
  3. Provide care promoting client/patient dignity, creating a calm and supportive atmosphere during each visit.
  4. Complete documentation in clinical records, time sheets, and quality control logs, upholding accuracy and attention to detail.
  5. Attend regular in-services to maintain and enhance professional development, staying current with industry best practices.
Requirements:
  • High school diploma or equivalent preferred, reflecting a solid educational foundation.
  • Six months to one year of experience in a related field desired, showcasing relevant skills and expertise.
  • Current Nursing Assistant License in New Hampshire required, demonstrating commitment to professional licensure.
  • Driver's License needed for travel throughout New Hampshire, ensuring flexibility and adaptability.
