Job Title: Pharmacist

We are seeking a highly skilled Pharmacist to join our team at Nuvance Health, a leading healthcare provider in the region.

Job Summary:

The Pharmacist will be responsible for ensuring the proper management of medications, including storage, preparation, dispensing, use, and administration, in accordance with accepted standards of medical, nursing, and pharmacy practice, as well as applicable hospital policy and law.

Key Responsibilities:
  • Medication Order Processing and Dispensing:
    • Reviews medication orders for completeness and appropriateness
    • Conducts patient profile reviews prior to dispensing
    • Detects and resolves basic order entry and prescribing problems
    • Makes retrospective, basic interventions to ensure work quality
  • Provision of Clinical and Drug Information:
    • Identifies problems and is aware of possible solutions/options
    • Retrieves information from available resources and provides general verbal and/or written drug information in an accurate and timely manner
    • Demonstrates understanding of basic pharmacotherapy of disease states
    • Collects and interprets data to design, recommend, implement, monitor, and modify patient-specific pharmacotherapy in collaboration with other healthcare professionals
  • Decentralized Pharmacist Role: Patient Teaching and Communication:
    • Actively participates in multi-disciplinary rounds to facilitate patient discharge or readmission prevention
    • Educates patients about their drug therapy to ensure safe medication use and optimize patient outcomes
    • Discusses patient needs with a team of physicians and nurses
  • Maintains Professional Competency:
    • Maintains current drug knowledge and keeps current with trends in pharmacy practice
    • Demonstrates initiative in learning about new disease and drug therapies
    • Represents the department and profession of pharmacy at committee or task force meetings and reports a professional summary back to the department manager and staff
    • Presents professionally in a multidisciplinary setting
  • Oversees/Checks/Monitors Pharmacy Technician Work:
    • Aware of and responsible for overseeing all activities pertinent to drug preparation, dispensing, and distribution by pharmacy technicians
    • Conducts in-process and final checks on the performance of all pharmacy technicians
    • Oversees and directs daily activities of pharmacy technicians
    • Maintains a teamwork and professional working environment
  • Project Management Skills to Extend Practice of Pharmacy:
    • Demonstrates ability to prioritize workload to optimize productivity or compliance with regulations
    • Develops, initiates, maintains, and completes assigned projects throughout the year
    • Works to improve or optimize assigned areas of responsibility
    • Documents compliance and coaches staff to standards
  • Provides Training, Team Teaching, Mentoring, and Precepting of Students:
    • Develops and uses training material to "on-board" new staff in all competencies
    • Precepts students in areas of rotation/concentration
    • Maintains skills and knowledge in new pharmacy laws, pharmacy practices initiatives, and cost savings ideas
  • Demonstrates Current Knowledge, Ability to Work, and Comply with All Pharmacy Regulations:
    • Ability to reference and look up pharmacy regulations
    • Ability to perform working knowledge of current regulations/requirements of pharmacy regulations
    • Ability to document and identify patterns of compliance or lack thereof
    • Ability to document, summarize, and report findings to the manager
  • Performs Other Duties as Assigned:
  • Fulfills All Compliance Responsibilities Related to the Position:
Requirements:
  • Bachelor of Science degree in Pharmacy from an approved college of Pharmacy
  • New pharmacy school graduate or limited experience (0-3 years of pharmacy practice experience)
  • Registered as a licensed pharmacist or eligible for reciprocity in the State of Connecticut
  • Must obtain a Connecticut pharmacist license within 3 months of employment
Preferred Qualifications:
  • Graduate from an ACPE-accredited pharmacy program (PharmD program or BS pharmacy)
  • Demonstrates knowledge, interprets, and complies with all state, federal, and malpractice laws of pharmacy practice
  • Demonstrates knowledge of USP 797, USP 800, and use of personal protective equipment
  • Contributes to teamwork with all team members and creating a positive culture
Computer Skills:
  • Able to use computer systems (PC, Microsoft Office: Word, Excel, PowerPoint)
  • Preferred: Cerner, Omnicell, MS Access, Simply797, Tracelink
Working Conditions:
  • Manual: significant manual skills/motor coordination and finger dexterity
  • Occupational: some occupational risk
  • Physical Effort: medium to heavy effort
  • Physical Environment: some exposure to dirt, odors, noise, human waste, etc.
