Key Responsibilities:

  • Collaborate with hospitals, rehabilitation facilities, and skilled nursing facilities to oversee patient inpatient stays and discharge planning.
  • Coordinate patient arrangements for both admissions and discharges.
  • Communicate effectively with internal and external stakeholders to uphold Bluestone's mission and ensure patient well-being.
  • Work towards minimizing unnecessary emergency department visits and reducing overall healthcare costs.
  • Enhance care coordination for a diverse and complex geriatric population.
  • Utilize partnerships to foster collaboration among acute and post-acute care settings.
  • Ensure patients receive appropriate care at the right time and place.
  • Be knowledgeable about community resources that support aging in place.
  • Proactively engage with healthcare providers to identify high-risk patients.

Qualifications:

  • Registered Nurse, Social Worker, or LPN with a four-year degree or equivalent experience.
  • 3-5 years of experience in value-based care, population health, case management, care coordination, or discharge planning is highly preferred.
  • A valid driver's license is required.

Skills and Abilities:

  • Ability to work independently and demonstrate strong customer service skills.
  • Excellent relationship-building and communication skills.
  • Proficient technical skills with experience in electronic health records preferred.
  • Alignment with Bluestone's mission and operational philosophies.
  • Willingness to travel occasionally within the service area as needed.
  • Proficient in reading, writing, speaking, and understanding the English language.
