Job Summary

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Senior Care Specialist. In this role, you will be responsible for providing high-quality nursing care to our residents, while also overseeing the delivery of nursing services and procedures.

Key Responsibilities

  • Develop and implement individualized care plans for residents, in collaboration with the Director of Wellness and RN.
  • Monitor and evaluate the quality of care provided by caregivers, and provide training and coaching as needed.
  • Assist with the onboarding of new caregivers, and ensure they are properly trained and equipped to provide high-quality care.
  • Conduct regular rounds to assess resident needs and provide support as needed.
  • Collaborate with the Director of Wellness and RN to ensure compliance with state and federal regulations, and to maintain high standards of quality care.
  • Communicate effectively with residents, families, and team members to ensure seamless care delivery.
  • Participate in team meetings and contribute to the development of care plans and policies.

Requirements

  • A current and unencumbered LPN license.
  • Strong computer skills and ability to interact with electronic devices.
  • Excellent communication and interpersonal skills.
