About the Role

Nationwide Children's Hospital is seeking a highly skilled Pediatric Care Coordinator to join our team. As a key member of our care coordination team, you will play a vital role in ensuring the highest level of care for our patients and their families.

Responsibilities
  • Assess Patient Needs: Conduct thorough assessments of patient and family strengths and needs to develop effective care plans.
  • Develop Care Plans: Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet the unique needs of each patient.
  • Coordinate Transitions: Manage transitions between settings, caregivers, and providers to ensure seamless care coordination.
  • Facilitate Communication: Facilitate communication and collaboration among healthcare teams to support the complex needs of patients and families.
  • Provide Education: Educate patients, families, and healthcare professionals on care coordination and case management best practices.
Requirements
  • Education: Bachelor's degree in Nursing required.
  • Licensure: Current licensure as a Registered Nurse in the state of Ohio required.
  • Certifications: Pediatric certification, Advanced-Continuity of Care Certification (A-CCC), or other Case Management certification desirable.
  • Skills: Excellent communication and collaboration skills, working knowledge of CMS and other review agency standards, and ability to relate to diverse age and demographic backgrounds.
  • Experience: Two years of nursing experience, with at least one year in pediatric nursing highly preferred.
