Job Summary:

Care Alliance is seeking a skilled Licensed Practical Nurse to join our Outreach Team. As a key member of our healthcare team, you will provide supportive medical care to clients who are homeless or living in Public Housing. You will participate in primary care clinics at Care Alliance sites throughout the service area, working collaboratively to foster a team approach in the management of client health.

Key Responsibilities:
  • Provide nursing care to clients, including venipuncture, monitoring of vital signs, and recording of medical information.
  • Work effectively with community service agency case managers and other healthcare professionals to ensure comprehensive care.
  • Perform triage/intake on all patients, expediting the daily schedule and patient flow.
  • Conduct basic health assessments, including vital signs and chief complaint, under the supervision of the Registered Nurse.
  • Implement appropriate components of the plan of care, under the direction of the healthcare team.
  • Assist the Podiatrist with various common office procedures and surgeries.
  • Schedule, review, and/or monitor referrals placed by the Podiatry Provider.
  • Collaborate with other Care Alliance health professionals to facilitate a team approach to patient care.
  • Assist with the development of nursing protocols and the orientation of new clinical personnel.
  • Maintain a safe, secure, and healthy work environment by establishing and following standards and procedures.
  • Participate in clinical program development, implementation, and evaluation.
Requirements:
  • Current license to practice as an LPN in the State of Ohio.
  • Good verbal and written communication skills.
  • Problem-solving skills.
Technology Requirements:
  • Basic understanding and familiarity with Office 365/Outlook and Epic.
Physical Requirements:
  • Normal medical/office environment.
  • Requires eye/hand coordination and manual dexterity sufficient to operate a computer keyboard.
  • Frequent contact with staff and patients.
  • May be stressful at times.
  • Thinking, walking, sitting, and/or standing for long periods.
  • Some bending and stretching required.
  • May require lifting papers or boxes up to 25-30 pounds.
  • Requires normal range of hearing and eyesight to record or prepare reports.
