About the Role:

As a seasoned clinical professional, you will play a pivotal part in managing participant onboarding and reassessment processes at Northwell Health. Your expertise will ensure accurate completion of the Uniform Assessment System of New York (UAS-NY) and effective collaboration with care delivery teams.

This role involves conducting comprehensive assessments, medication reviews, and post-hospital discharge evaluations while maintaining compliance with Department of Health (DOH) policies and regulations.

Key Responsibilities:

  • Conduct in-home or virtual UAS-NY clinical assessments for prospective and enrolled PACE participants
  • Review and update previous assessments, primary diagnoses, and medications in electronic medical records (EMR)
  • Collaborate with Intake, Enrollment, and Care Delivery teams to facilitate smooth transitions and optimal care for participants
  • Evaluate timely documentation completion within 48 hours and maintain accurate clinical records
  • Engage in continuous quality improvement initiatives and provide feedback to enhance processes and participant outcomes
  • Serve as the clinical point of contact for Care Delivery teams and offer ongoing support after participant enrollment
  • Maintain compliance with all CMS/PACE guidelines and organizational policies

Requirements:

  • Bachelor of Science in Nursing (BSN) preferred
  • 3-5 years of experience in Managed Care Programs (MLTC, LTHHCP, MAP, PACE, etc.)
  • 5+ years of experience in case management, disease management, utilization review, or quality assurance
  • Bilingual skills are strongly preferred
  • Proficiency in Microsoft Office and clinical management systems

Salary: The estimated salary range for this position is $120,000 - $150,000 per year, based on the location and industry standards.
