Care Manager

We are seeking a highly skilled Care Manager to join our team at Nebraska Total Care. As a Care Manager, you will play a critical role in developing and implementing care plans that improve health outcomes for our members.
Key Responsibilities- Assess Member Needs: Evaluate the needs of our members, identify barriers to care, and develop personalized care plans to address their unique needs.
- Collaborate with Providers: Work closely with healthcare providers to ensure our members receive timely and effective care.
- Coordinate Care: Coordinate care between members, caregivers, and healthcare providers to ensure seamless transitions and optimal health outcomes.
- Monitor Progress: Regularly monitor member progress, identify areas for improvement, and revise care plans as needed.
- Provide Education and Support: Educate members and their families on disease management, healthcare options, and available resources.
- Improve Quality and Efficiency: Identify opportunities to improve care quality and efficiency, and implement changes to enhance member outcomes.
- Education: Bachelor's degree in Nursing or related field.
- Experience: 2-4 years of experience in care management or a related field.
- Licensure: RN - Registered Nurse - State Licensure and/or Compact State Licensure required.
- Competitive Pay: $54,000 - $97,100 per year.
