We are seeking a highly skilled Primary Care Physician to join our team at CenterWell Senior Primary Care in West Vine. As a Primary Care Physician, you will play a vital role in providing proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment.
Key Responsibilities- Provide high-quality, patient-centered care to seniors in a fast-paced, dynamic environment.
- Develop and implement personalized care plans to address the unique needs of each patient.
- Collaborate with a multidisciplinary team of healthcare professionals to ensure comprehensive care.
- Stay up-to-date with the latest medical research, guidelines, and best practices to provide evidence-based care.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Doctorate degree in Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) from an accredited university.
- 5+ years of experience in Value-Based Care/Managed Care.
- Licensure requirements in the State of Florida.
- Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Preferred: Bilingual (Spanish/English) speaking.
