Job Summary:

We are committed to delivering exceptional care and compassion to our patients and their loved ones. As an On Call RN, you will be responsible for providing on-call coverage, conducting scheduled after-hours visits, and collaborating with RN Case Managers to ensure seamless patient care.

Responsibilities:
  • Represent Empyrean Hospice with professionalism and compassion
  • Conduct comprehensive assessments of patients and families to determine hospice needs
  • Develop and implement individualized care plans that prioritize patient comfort and well-being
  • Coordinate patient care for assigned caseloads
  • Counsel patients and families on meeting nursing and related needs
  • Provide health education and support to patients and families
  • Collaborate with physicians and hospice care teams to ensure continuity of care
  • Ensure a safe environment for patients and arrange necessary equipment and services
  • Supervise ancillary personnel and delegate responsibilities as needed
Requirements:
  • Current licensure in State and CPR certification
  • Graduate of an accredited school of nursing with 1-2 years of recent acute care experience
  • 1-2 years of recent experience in home care
  • Bachelor's degree, with one year of hospice care experience preferred
  • Excellent observation, verbal and written communication skills, problem-solving skills, and basic math skills
  • Able to lift, position, and/or transfer patients, and able to lift supplies and equipment
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
