The RN Care Manager is responsible for executing care management functions to evaluate and synchronize all facets of medical and ancillary services for individuals with complex and high-acuity health requirements. This role is essential in fostering quality and cost-effective healthcare delivery. The RN will create tailored care plans for long-term care members, address their needs, and provide education to members and their families regarding available services and benefits to ensure they receive optimal care.

Key Responsibilities:

  • Conduct comprehensive in-home evaluations, which include:
  • o SAI Core A to Core Q
  • o ISP
  • o EDP
  • o Additional necessary forms: 1500 forms, SBP, EVV
  • o Evaluate the member's current health condition, resource usage, previous and ongoing treatment plans, and both short- and long-term objectives.
  • o Follow up on requests for necessary items/services.
  • o Communicate with healthcare providers.

Performance Metrics:

  • Achieve a minimum of 6 successful visits weekly.
  • o Submit assessments within 24 hours post-visit.
  • o Complete all other documentation within 5 business days.
  • Visits are generally scheduled Monday through Friday, 8 AM to 5 PM; however, due to contractual obligations, timely submission of assessments and documentation is required.
  • Due to the pediatric demographic, some visits may occur after 3 PM, and there may be requests for Saturday visits.
  • Home visits are essential, necessitating travel to members' residences.
  • Travel may extend beyond a 100-mile radius.
  • o Preferred counties include:
  • Hockley, Lubbock, Crosby, Terry, Lynn, Garza, Lamb, Hale, and Floyd.
  • o Visits will occur within the designated service area, encompassing counties from Hutchinson to Terry, Lynn, and Garza.
  • o Visits may extend to surrounding counties.
  • Travel time will be factored into productivity assessments for the week.
