The primary responsibility is to facilitate smooth transitions from hospital to home or other care settings, integrating care coordination, utilization management, and discharge planning.

Key Responsibilities

  • Serve as a communication bridge between primary care teams and inpatient staff to promote effective collaboration.
  • Ensure timely transitions for patients moving from hospital care to their homes or other post-acute facilities, fostering a continuous care experience.
  • Keep primary care providers informed about patient progress and any modifications in treatment strategies.
  • Organize necessary outpatient tests and follow-up appointments to support ongoing patient care.
  • Continuously assess the care plan to ensure objectives are being met and make adjustments as needed.
  • Work closely with emergency department personnel to guarantee appropriate follow-up care is arranged prior to patient discharge.
  • Advocate for patients by addressing challenges in care coordination, including medication management and access to necessary services.
  • Engage in interdisciplinary meetings to develop comprehensive, patient-focused care plans.
  • Maintain accurate documentation of all care management activities.
  • Collaborate effectively with medical and nursing teams.
  • Support the organization's mission and values through staff education initiatives.
  • Ensure thorough documentation within care management systems.
  • Participate in quality improvement programs.
  • Oversee the care coordination process executed by Care Coordinators.
  • Commit to ongoing professional development.
  • Collaborate with healthcare providers and leadership to ensure coordinated service delivery.
  • Contribute to departmental quality improvement processes.
  • Perform additional duties as required.

Qualifications

  • Current and valid RN license in the applicable state(s) or a compact license.
  • A minimum of 3 years of clinical experience with a solid understanding of patient care processes in inpatient and emergency settings.
  • Exceptional communication skills, both written and verbal, enabling effective interaction with patients and healthcare professionals.
  • Strong organizational skills with the ability to manage multiple tasks and meet deadlines.
  • Knowledge of care coordination, transitional care, and discharge planning principles.
  • Proficient in electronic health record (EHR) systems and relevant software.
  • Demonstrated accountability and independence in work.
  • Able to prioritize critical issues in a diverse workload.
  • Capacity to influence and drive positive change.
  • Strong analytical and critical thinking abilities.
  • Current knowledge of disease processes and psychosocial factors affecting treatment.
  • Effective time management skills.
  • Positive, service-oriented demeanor.
  • High integrity and professionalism.
  • Ability to maintain a HIPAA-compliant work environment.
  • Certification in Care Management is highly desirable.
