The role involves executing care management responsibilities to evaluate and synchronize all facets of medical and ancillary services for complex and high-acuity patient populations with primary health needs, ensuring the delivery of quality and cost-effective care. The RN will formulate individualized care plans for long-term care clients, addressing their needs and educating them and their families or caregivers about available services and benefits to facilitate access to high-quality care.

Key Responsibilities:

  • Conduct comprehensive in-home evaluations, which include:
  • o SAI Core A to Core Q
  • o ISP
  • o EDP
  • o Additional forms as necessary: 1500 forms, SBP, EVV
  • o Evaluating the client's current health status, resource usage, previous and ongoing treatment plans, and setting short- and long-term objectives, along with treatment and provider alternatives.
  • o Follow up on requests for services/items
  • o Communicate with healthcare providers

Performance Metrics:

  • Achieve 6 successful visits weekly
  • o Submit assessments within 24 hours post-visit
  • o Complete all other documentation within 5 business days
  • Visits typically occur Monday through Friday, 8 AM to 5 PM; however, due to contractual obligations, assessments and documentation must adhere to specified turnaround times.
  • Due to the pediatric demographic, some visits may occur after 3 PM, and there may be requests for Saturday visits.
  • Visits are conducted in clients' homes, necessitating travel to their locations.
  • Travel may extend beyond a 100-mile radius.
  • o Preferred counties include:
  • Hockley, Lubbock, Crosby, Terry, Lynn, Garza, Lamb, Hale, and Floyd
  • o Visits will be within the Lubbock Service Delivery Area, covering counties from Hutchinson to Terry, Lynn, and Garza
  • o Visits may also include adjacent counties
  • o Travel time will be factored into productivity assessments for the week.
