Compensation Overview:

Total Care Services, Inc. is in search of compassionate and committed individuals to fill the role of Community Services Coordinator within our Coordination of Community Services program.

In this position, you will be instrumental in assisting individuals with intellectual and developmental disabilities and their families in Maryland.


Your Responsibilities Include:
  • Collaborating with clients and their families to assess needs, aspirations, and preferences.
  • Providing guidance on community resources and available support services.
  • Empowering clients to make informed decisions and strategize for their future.
  • Facilitating access to service delivery systems, resources, and supports.
  • Monitoring and evaluating services to ensure alignment with clients' needs and preferences.

Work Environment:
This is a hybrid role, allowing you to work from your home office while conducting local community visits.

Provided Equipment:
Laptop, printer, and cell phone.

Work Schedule:
8:30 AM - 5:00 PM, Monday through Friday (flexible as required).

Qualifications:
We are looking for Community Services Coordinators who are:
  • Dedicated to supporting our clients and their families.
  • Strong advocates for individuals with intellectual and developmental disabilities.
  • Experienced in Case Management.
  • Capable of entering case notes and achieving a minimum of 7.5 billable hours daily into LTSSMaryland.
  • Possessing excellent written and verbal communication skills.
  • Highly organized with strong time management abilities.
  • Technologically proficient.
  • Experienced in Community-Based Services Case Management.
  • Holding a Bachelor's degree in a human services field such as psychology, social work, sociology, nursing, counseling, or criminal justice.
  • A licensed driver with reliable transportation, valid insurance, and a safe driving record.
  • Fully vaccinated and able to meet all pre-employment qualifications.
