Key Responsibilities
  • Assess the needs of elderly clients and coordinate the delivery of home/personal care services.
  • Work closely with the interdisciplinary team to develop and implement care plans that meet the unique needs of each client.
  • Provide technical, organizational, and communication skills to ensure efficient and effective care delivery.
  • Collaborate with healthcare professionals to ensure seamless care transitions and optimal client outcomes.
  • Conduct home visits to assess client needs and provide care coordination services.
  • Develop and maintain relationships with clients, families, and healthcare providers to ensure comprehensive care.
Requirements
  • Graduate of an accredited Certified Occupational Therapist Assistant (COTA) program.
  • At least one year of experience working with a frail elderly population, preferably in long-term care, home care, or community health.
  • Valid PA COTA license and valid PA Driver's license.
  • Strong organizational and planning skills, with the ability to manage multiple priorities.
  • Effective written and oral communication skills, with the ability to work independently and utilize critical decision-making skills.
