Key Responsibilities
- Provide services ordered by physicians, including counseling and education to patients and families
- Collaborate with healthcare teams and community agencies to identify patient needs and provide referrals to community resources
- Record patient progress and report to physicians, participating in quality assessment and training programs
Requirements
Minimum 3 years of experience in home healthcare, with a license in Clinical Social Work and a Master's Degree
Working Conditions
- Little to no manual skills required, with some exposure to occupational risks
- Heavy physical effort may be needed, with exposure to various environmental factors
