Responsibilities
  • Collaborate with healthcare professionals to develop and implement patient care plans

  • Manage patient flow and coordinate transitions of care
  • Identify and address patient needs and concerns
  • Develop and maintain relationships with community resources and healthcare providers
Requirements
  • Current New York State registration as a Registered Professional Nurse
  • Bachelor of Science degree in Nursing required within five years of hire
  • 2 years of acute care nursing experience
  • Knowledge of Interqual Criteria set and software systems preferred
  • Working knowledge of Microsoft Office Suite and Outlook
What We Offer
  • Competitive hourly rate: $42.09-$53.86 per hour
  • Variable shift and scheduled work hours
  • Per diem position with no weekends or holidays required
