Responsibilities- Collaborate with healthcare professionals to develop and implement patient care plans
- Facilitate communication between patients, families, and healthcare providers
- Manage patient flow and coordinate transitions of care
- Identify and address patient needs and concerns
- Develop and maintain relationships with community resources and healthcare providers
- Current New York State registration as a Registered Professional Nurse
- Bachelor of Science degree in Nursing required within five years of hire
- 2 years of acute care nursing experience
- Knowledge of Interqual Criteria set and software systems preferred
- Working knowledge of Microsoft Office Suite and Outlook
- Competitive hourly rate: $42.09-$53.86 per hour
- Variable shift and scheduled work hours
- Per diem position with no weekends or holidays required
