Nurse Coordinator

As a Nurse Coordinator - Patient Care, you will play a vital role in coordinating total patient care through assessing, planning, implementing, and evaluating patient needs. You will participate in patient and family teaching, providing leadership by working cooperatively with the healthcare team to maintain standards for professional practice in the clinical setting. You will also participate in quality improvement initiatives to ensure the highest level of patient care.
Responsibilities:
* Assess patient needs and develop individualized care plans
* Coordinate patient care with the healthcare team
* Participate in patient and family teaching
* Provide leadership and guidance to the healthcare team
* Participate in quality improvement initiatives
Requirements:
* Graduate of an accredited School of Nursing
* Current New York State license or Permit
* Current BLS certification or obtained within 4 months of hire
* Meets and is current and competent with all New York State licensure requirements, not limited to Infection Control and Child Abuse mandatory reporting
Working Conditions:
* Subject to inside environmental conditions
* Occasional exposure to one or more mildly unpleasant physical conditions
