Join Our Team as a Registered Nurse

We are seeking a dedicated and compassionate Registered Nurse to join our team at Transition Home Healthcare. As a Registered Nurse, you will play a vital role in delivering high-quality patient care and services to our clients in the Houston, TX area.

Responsibilities:
  • Oversee the nursing care team and ensure seamless coordination of patient care.
  • Collaborate with physicians, therapists, and other healthcare professionals to provide comprehensive patient care.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Educate patients and their families on managing their health conditions, medication regimens, and lifestyle changes.
  • Prepare accurate and timely clinical notes to ensure continuity of care.
Qualifications:
  • Valid RN license in the State of Texas.
  • At least 1 year of home health experience.
  • Knowledge of home health regulations and general healthcare compliance.
  • Experience with Home Care Home Base (HCHB) EMR is preferred but not required.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
