Care Coordinator Job Description

We are seeking a skilled Care Coordinator to join our team at Texas Children's Hospital. As a Care Coordinator, you will play a critical role in ensuring the optimal patient outcomes by coordinating and managing care across the continuum of care.

Key Responsibilities:
  • Assess and develop comprehensive plans of care through an interdisciplinary team process in conjunction with patients and families.
  • Collaborate with medical and nursing staff to ensure smooth facilitation of level of care and patient placement.
  • Identify and mitigate barriers to implementing plans of care through effective communication and coordination with the care team.
  • Ensure accurate and timely documentation of patient information in the medical record.
  • Participate in the orientation and training of new department members and identify quality improvement opportunities.
Requirements:
  • Bachelor's Degree in Nursing or an Associate's degree in Nursing with enrollment in a BSN program.
  • 3 years of nursing experience in an acute care setting, with a focus on community health, complex pediatric patient care, home care, case management, managed care, or utilization review.
  • Certified Care Coordinator and Transition Management by Medical-Surgical Nursing Certification Board (CCTM).
Preferred Qualifications:
  • Excellent verbal and written communication skills, with the ability to operate a personal computer, word processing programs, fax machine, photocopier, and multi-line telephone.
  • Ability to work effectively in a fast-paced environment, with a focus on patient-centered care and quality outcomes.
