Peer Support Specialist

Family Service League Inc. is seeking a Peer Specialist to join our Center of Treatment Innovations in Hauppauge. The successful candidate will provide in-person and telephone-based support, coordination of care, and linkages in the community and onsite at the 24/7/365 Crisis Stabilization Center and an Outpatient SUD program.

Key Responsibilities- Provide recovery-based support to facilitate engagement in assessment and services.
- Work as part of a multidisciplinary team providing crisis assessments in the community, via telehealth, and onsite at the crisis stabilization center.
- Provide warm hand-off linkage to aftercare and follow up with clients to monitor engagement.
- Complete all documentation in an electronic medical record (EMR).
- Coordinate with outside providers, collateral contacts, and other stakeholders.
- Attend scheduled community events and engage in community outreach to individuals in need of support and services.
- Participate in routine supervision/team meetings.
- Complete training and maintain proficiency with de-escalation and crisis intervention techniques.
- Establish and maintain positive working relationships with others both internally and externally to achieve the goals of the organization.
- Communicate effectively, both verbally and in writing.
- Foster teamwork, work cooperatively, and effectively with others to set goals, resolve problems, and make decisions.
- Assess situations to determine importance, urgency, and risks, and make clear decisions that are timely and in the best interests of the organization.
- Set priorities, develop a work schedule, monitor progress towards goals, and track details, data, information, and activities.
- High school diploma or equivalent required.
- Peer Certification, Provisional Certification (CRPA, CRPA-P), or willingness to become certified required.
- Prior experience as a peer, paid, or volunteer preferred.
- Relevant life experience required and willingness to draw upon this experience to engage individuals and serve as a role model required.
- Proficient computer skills, including Microsoft Office required; experience with EHR preferred.
- Bilingual in Spanish preferred.
- Valid and clean NYS Driver's License required.
- Some evening and weekend hours required. Some holiday shifts required.
